Acute and chronic changes in LV performance in patients with primary severe mitral regurgitation undergoing percutaneous mitral valve repair

Abstract Background The non-invasive assessment of myocardial work allows to evaluate LV performance by taking into account the effect load. Aim this study was acute and chronic impact percutaneous mitral valve repair (PMVR) on performance, in patients with severe primary regurgitation (PMR). Methods 71 (age: 77±9 years, females: 44%) PMR (effective regurgitant orifice: 0.57±0.31 cm2, volume: 80±34 ml) preserved ejection fraction (LVEF: 60±10%) underwent PMVR according current recommendations. Myocardial indices were evaluated before procedure, 2 days after procedure at 1-year follow-up. Results associated an reduction LVEF, global longitudinal strain (GLS), index (MWI), efficiency (MWE), a significant increase wasted (MWW) mechanical dispersion (MD). size significantly decreased, GLS, MWI, MWE MD improved, whereas GWW remained reduced. LVEF improved but altered compared baseline values (Table 1). Conclusions In undergoing PMVR, preload causes impairment all parameters performance. After 1-year, progressive remodeling is improvement MCW, counterbalanced persistently high MWW, which can explain partial recovery LVEF. Funding Acknowledgement Type funding sources: None.
